We participate in most major insurance plans, and our office will check your coverage with you before your first visit rather than leaving you to work it out.

Medicare

Medicare Advantage
- Florida Blue — the BlueMedicare PPO plans, including Select, Value, Patriot and Patriot Plus, and BlueMedicare Group PPO for employer retirees
- Humana — the HumanaChoice Florida PPO plans, Humana Full Access Giveback, Direct Choice Giveback, Value Choice and USAA Honor Giveback
- WellCare — Giveback, Patriot Giveback and Simple
Plans for people with both Medicare and Medicaid

Plan networks change, and this list is updated as contracts change. If you do not see your plan, call us on (239) 415-4900 and we will check it for you before you book.
If your plan is one we do not participate in, call us anyway. We would rather spend two minutes helping you find someone who does than have you drive out here and be turned away.
You may still see us listed under our former name, Cardiology Consultants, in your plan's provider directory. It is the same practice, the same physicians and the same address. Directories take a while to catch up. If that is confusing, call us and we will sort it out with you.
How to check your own coverage in two minutes
- Call the number on the back of your insurance card and ask whether our practice and our physicians are in network for your specific plan. Ask them to note the reference number of the call.
- Have your plan name and member ID ready. Two plans from the same insurer can have different networks, so the plan name matters as much as the insurer.
- Or call us and we will check it with you.
What to bring to your first visit
- Your insurance card and a photo ID
- A list of your current medications, including anything bought over the counter
- Any recent test results, and actual images on a disc if you have them
- A referral, if your plan requires one. We can tell you whether yours does.
Questions about cost
If you would like an estimate before you come in, ask us. We would rather have that conversation in advance than have you receive something unexpected afterwards.
